Its that time of year when we have to fill out a form for merit pay.  Decisions on merit pay are based (presumably) on what we've done this past year in terms of teaching, research, service, etc...  Below the fold is the information that's going on my merit pay summary form; it gives a pretty good overview of what I've been up to, professionally  speaking, this past year...
